Alternative Health Care

Alternative Health Care

The Alternative Health Care (AHC) Program facilitates the transition from hospital to home, supporting clients in returning to independent living. AHC is a 3 month program for individuals that have discharged from a hospital, nursing home, or rehabilitation center within the last 30 days. If the individual needs care following the 3 months, and they meet the HHHA financial criteria, they can transition to the ongoing HHHA program.

For more information about our AHC program, please reach out to our office at 702-455-4270, Monday through Friday from 8:00 am to 5:00 pm. You can also contact us via email at AdultCareServices@ClarkCountyNV.Gov. We are here to assist and support you in fostering a secure and independent living environment.
