Requirements
- Submit a W9
- Provide copies of all current business licenses and certifications.
- Provide copies of emergency evacuation procedures.
- Pass initial and regular facility site inspections conducted by CCSS staff.
- Maintain personnel files for all facility staff, including status information to document employability according to the DHS.
- Overall cleanliness appearance of the facility/residence.
- Complete purchasing required documents (if applicable)
