The Community Navigator played a crucial role in addressing her needs by working closely with her grandmother and mother. They managed to re-enroll Jaylen in a program tailored to her specific requirements, which included making sure her medical appointments were up to date. As a result, Jaylen was able to thrive academically and attend school consistently without any missed days after her re-enrollment. This positive outcome highlights the impact of personalized support and community intervention in helping students overcome obstacles and succeed.

In this TPOP case, the student, who had frequently moved and thus missed out on crucial assessments and accommodations, finally had the opportunity to receive the support they needed. At 9 years old, an Individualized Education Program (IEP) was completed with the help of a Community Navigator. The Navigator also facilitated curbside transportation for the student, which contributed to improved attendance. This intervention made a significant difference in the student’s educational experience and overall well-being, ensuring they now have the accommodations and support necessary for their success.
